
Approaches and Challenges to Optimizing the Use of the Electronic Health Record in Primary Care

Abstract
This qualitative study investigates the approaches to electronic health record (EHR) quality improvement that optimization analysts took and the challenges that they perceived in their work in a large Midwestern academic healthcare system. Findings suggest that approaches to EHR optimization varied at the levels of the organization, primary care teams, and team members. A major challenge was the tension between standardization required by the organization and the customization desired by the empowered primary care teams. Other challenges identified at the team level included a reluctance to change existing habits, a lack of dedicated time for optimization, and a lack of technological knowledge. The implications of this study highlight the need to involve different stakeholders’ opinions in the process of EHR optimization planning. An in-depth understanding of the workflow, the cognitive and interactional activities in primary care is necessary for successful EHR optimization.
